I had quite a few requests for the dispensing visit form I mentioned in last week’s tip article, so I will provide it here. If you missed my column last week, here is a link to it: Tip 776.
The key point in using this paper record form is to make it easy for your staff to obtain patients’ names when they drop in for a repair or adjustment. These forms are in pads of 100 with a cardboard back and each patient can print his own name at the top. The size of the paper is 4.25 X 5.5 inches.
The staff can then look up the last exam date and the optician can let the patient know that he is past due for eye care and recommend scheduling an appointment right now. Other useful data can be collected and the form can be scanned into the EHR system.
